You will be asked the reason for your appointment by our highly trained reception staff, who will be able to best advise and direct your appointment to the most appropriate clinician. You may be signposted to another service such as the local pharmacy under the Common Ailment Services, cluster Physiotherapist for acute pain, cluster Mental Health Liaison Practitioner, Practice Pharmacist or other services
Appointments are still being offered via telephone with the appropriate clinician and a face to face appointment will be offered if clinically indicated or requested.
Appointments with a GP can be pre-booked up to four weeks in advanced. You can contact the surgery at anytime to book these appointments. We have made a few changes to make it clear that patients should be offered an assessment of need, or signposted to an appropriate service, at first contact with the practice. One of our friendly and highly trained receptionists will ask you for the reason for the appointment, so that you can be directed you to the most suitable clinician. We want to see an end to the 8am bottleneck where patients have to telephone their practice numerous times day after day to get an appointment.
An appropriate appointment with the most suitable clinician will be arranged for you. Please be aware we are unable to give you a set time for telephone consultations, and your call will either be during either our morning surgery (9am - 12pm) or our afternoon surgery (2pm - 6.30pm).
If you are asked to attend the practice, you will be given a specific time. Please ensure you use the self check in on arrival and wait to be called by the clinician from our main reception. Please not that if you arrive more than 5 minutes late for your appointment, you will be asked to rebook.
All clinics and nurse/HCA appointments are available to book up to 4 weeks in advance, subject to availability. A number of appointments are made available to book on the day, however it is recommended that these appointments be booked in advance.

To request a non-urgent consultation, you can send your request via email (include full patient details eg. name, date of birth, and contact details along with the reason for consultation) to our email enquiries.w93125@wales.nhs.uk. A receptionist will contact you with an appointment. Please note we are not always able to honour requests for a specific clinician which may be due to leave or regular days of work.
We offer annual disease monitoring for patients with Chronic diseases such as Diabetes, Asthma and Hypertension. You will be sent an invitation the to book your review, which will include a full review of your medications, with the appropriate clinician. You may be required to complete a blood test prior to your review, as stated in your letter.
The practice will continue to invite patients for annual review. The invite may include a questionnaire to complete and return prior to your review. This will allow the clinician to review and send you a text to either advise that:
The review may include lifestyle, medication review to optimise the treatment of the condition, blood tests, urine samples and monitor for any complications.
Not all chronic diseases are reviewed by the doctors. The majority of reviews are more appropriately carried out by the practice Pharmacist or the Practice Nurse. The reception team will book your appointment with the most appropriate clinician.
Clinical Pharmacists work as part of the general practice team to resolve day-to-day medicine issues and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medications and better access to health checks. The role is pivotal to improving the quality of care and ensuring patient safety.
Having a clinical pharmacist in GP practices means GPs can focus their skills where they are most needed, for example on diagnosing and treating patients with complex conditions. This helps GPs manage the demands on their time.

Emma Jones is our Independent Prescriber who manages medication reviews, chronic disease reviews and other queries e.g. pain medication, medication synchronisation and reconciliation. Emma is also trained in Minor Illness and offers appointments Tuesday to Friday. You can pre-book these appointments up to four weeks in advanced.
