Care Navigation 

Please note when booking an appointment , you will be asked by a receptionist for a brief description of your symptoms. All receptionist are trained in care navigation. 

What is Care Navigation

Care Navigation involves a  receptionist supporting patients by signposting them to the most appropriate professional or service. This is being implemented to help patients ensure they receive the right care, first time and as efficiently as possible. This includes signposting you to a person or service within the practice as well as other NHS organisations, social care and in the community.

Telephone 

We run an appointment system Monday - Friday for Doctors,Nurses and other health professionals. 

You can telephone the surgery from 8am - 6:30pm to book an appointment on 01633 413258. 

Patients will be offered an appropriate consultation, whether urgently or through our advanced booking consistent with the patient's assessed clinical need, without the need for the patients to contact the practice again. A care navigator will ask you to give a brief description of your symptoms to enable them to guide you to the most appropriate health professional. 

We offer a mix of remote,face to face,urgent on the day and pre-bookable appointments. 

Online 

You can request a call-back to make a non-urgent appointment via email to enquires.w93125@wales.nhs.uk. Please include as much information as possible about the reason for your request. 

This email is for requesting non-urgent consultations or call backs . You will receive a telephone call within 24 hours from a receptionist in response to your enquiry. Please note that this email is for NON-urgent issues only. For urgent issues please contact reception on 01633 413258. Requests for repeat prescriptions will also not be actioned via this email. Please order your repeat prescriptions in your normal manner.

Please be advised that internet email accounts are not guaranteed as secure. There is always a risk of email accounts being intercepted or hacked. The practice cannot be held responsible for intercepted emails sent from a patients shared email account.

NHS Wales APP

Appointments online - We have currently turned this function off due to care navigation. We are unable to monitor appropriate appointments using this feature. Please be aware that we are looking into this and we are hoping to have this service up and running very soon. In the mean time,If you prefer to book appointment using an electronic system, please contact us on our enquiries email address above. This is for non-urgent appointments only.
